About Direct Primary Care
What is Direct Primary Care?
Direct Primary Care is a healthcare model where you pay a flat monthly membership fee directly to your physician with no insurance billing in between. In exchange you get unlimited access to your doctor, same or next day availability for urgent concerns, 30 to 60 minute visits, and a physician who actually has time to know you.
Can I keep my existing insurance?
Yes. A DPC membership works alongside your existing insurance. Most members pair their membership with a high-deductible health plan that covers hospitalizations, major procedures, specialist care, and emergencies. Your Impact membership handles primary care at a fraction of the usual cost.
Is my membership HSA or FSA eligible?
DPC membership fees are generally eligible for reimbursement through Health Savings Accounts and Flexible Spending Accounts. We recommend confirming with your plan administrator. If you need help choosing an insurance plan that works alongside your DPC membership, we can connect you with an independent broker who specializes in exactly this.
What is the difference between DPC and Concierge?
The right membership comes down to three things: your budget, whether you want to participate in the Impact 100 longevity program, and how much direct physician access matters to you day to day. DPC is excellent proactive primary care at $150 per month with same or next day urgent access and direct messaging to your care team. Concierge adds direct physician texting with response within 24 hours any day of the week, same day visits, and full enrollment in the Impact 100 longevity program. Not sure which fits? What if I need to see a specialist?
We coordinate referrals to specialists and work closely with them to make sure your treatment plan is clear and followed through. Concierge members receive an additional layer of physicianled coordination to keep everything connected.
What are your membership terms? Can I cancel?
Memberships are billed quarterly or annually. You are free to cancel at any time after your first quarter with no long term commitment required. We want you to stay because the care is worth it, not because you feel stuck.

About Your First Visit
What does the first visit look like?
Your first visit will depend on the program you have enrolled in, but here is what you can generally expect.
Whether in person at our Heber City clinic or via telehealth, you will begin by confirming your information with our front desk staff. Before your visit you will have already completed your intake forms through the Impact patient portal, which keeps your appointment focused on you rather than paperwork.
A medical assistant will record your vitals and review your past medical history, current medications, and pharmacy information. You will then meet with your physician for a 30 to 60 minute visit focused on your goals, your health concerns, and the program you have enrolled in. You will leave with a clear next step and a physician who knows your name and your story.
Do I need to fill out forms before I come in?
Yes. After you enroll you will receive a prompt to log into the Impact patient portal to complete
your intake forms. Please do this before your first visit so we can make the most of your time
with your physician.
Can I do my first visit over telehealth?
Yes. Initial visits are available via telehealth for most programs. Services like DEXA scanning and VO2 max testing are available at our Heber City clinic and at select locations across Utah.

About Concierge Membership
What does Concierge include that DPC does not?
Concierge membership includes full enrollment in the Impact 100 longevity program which covers your DEXA scan, VO2 max testing, advanced blood panel, and the full physician-led risk assessment and care planning process. It also includes direct physician texting with response within 24 hours any day of the week, same day visits, close specialist coordination, hospitalization support, travel medicine, priority scheduling for your family, and in-home visits for an additional fee.
How does after-hours access work for Concierge members?
Concierge members have direct access to their physician around the clock. You will always hear back within 24 hours any day of the week. We trust our patients to use that access thoughtfully, and we trust ourselves to triage quickly and guide you to the right level of care, whether that is a message back, a telehealth call, or a recommendation to head to urgent care or the ER. We will always tell you honestly which one you need.
Are house calls available?
Yes. In-home visits are available to Concierge members. Contact us for details.
About Telehealth
Is telehealth available?
Yes. Impact Medical offers telehealth visits for established patients across the state of Utah. You can see your physician from wherever you are. Some diagnostic services including DEXA scanning and VO2 max testing are available at our Heber City clinic and at select locations across Utah.
About Weight Loss and GLP-1 Medications
Do you prescribe GLP-1 medications like Ozempic or Mounjaro?
Yes. Our board certified obesity medicine specialist evaluates each patient individually and prescribes FDA-approved weight loss medications when clinically appropriate as part of a comprehensive treatment plan. This includes Ozempic and Wegovy (semaglutide), Mounjaro and Zepbound (tirzepatide), and other FDA-approved options. Medication is always part of a broader plan that includes nutrition, exercise, and behavioral support.

What are your hours?
We are open Monday through Friday 8am to 5pm. Concierge members have direct physician
access outside of clinic hours.
